Teresa Pearson, MS, RN, CDE, FAADE, explains what a “medical home” is, and how the concept has enabled Minnesota to deliver more patient-centric healthcare, with a whole team of medical practitioners coordinating care for each patient. In the current norm of healthcare, it is often up to the patient to connect all the dots in the system; with a medical home system, a patient coordinator fulfills this function. Payers have moved towards this system reluctantly, and Pearson says that there are definitely challenges in implementation, but that it is worth it because of the improvements seen in patient health.
