The number of GLP1-RA’s has grown over the past few years, and the fact that there are still new ones working their way towards approval can be viewed as a measuring stick to how important this class will be for treating diabetes now and going forward. Each time a new class member comes out, the manufacturer is forced to find a way to differentiate itself from the rest of the class. This could be as simple as a better delivery device, or looking for new benefits in patient care. This week our intern Alan Martinez, PharmD Candidate, USF College of Pharmacy, takes a look at Efpeglenatide, the newest GLP1-RA, and the unique benefits for cardiovascular and renal protection, in our Laterpay feature.

People with diabetes are 2 to 4 times more likely than others to develop cardiovascular disease. Because this risk is so high, cardiovascular disease remains the most common cause of death in people with diabetes, with more than 50% of deaths coming from CVD.
A study in Diabetes Care looked at the potential impact of instituting earlier glucose control compared with delayed glucose-lowering therapy. They found that patients with type 2 diabetes who maintained a one percentage point lower A1C from the time they were diagnosed had an 18.8% decreased risk for all-cause mortality 10 to 15 years later, while those who waited ten years after diagnosis to maintain a lower A1C had a 2.7% reduced all-cause mortality risk. Maintaining a lower A1C level shortly after diagnosis also was associated with greater reductions in risk for myocardial infarction. The glycemic legacy effects seen in type 2 diabetes are mainly explained by historical HbA1c values having a greater impact than current clinical outcomes. Early detection of diabetes and intensive glucose control from the time of diagnosis is essential to maximize the reduction of the long-term risk of glycemic complications. Maybe it is time for the ADA to recommend pharmaceutical help for those with prediabetes. and lower the diagnosis of diabetes from 7% to 5.7%? Diabetes Care June 2021
