Issue 171 Item 11 Sustained Release Indapamide Helpful in Diabetic Hypertensives

Sustained release (SR) indapamide lowers blood pressure in diabetic hypertensives without affecting glucose metabolism or having other untoward effects,

Dr. Shi-Wen Kuo and colleagues at Tri-Service General Hospital, Taipei note that thiazide diuretics are widely used anti-hypertensives but they may alter lipid profiles and affect glucose control. Indapamide, a thiazide derivative was developed in order to minimize such effects.
To evaluate indapamide SR, the researchers randomized 64 type 2 diabetic patients with mild-to-moderate hypertension to daily treatment with 1.5 mg of the agent or to placebo.

 

Over 3 months of therapy, standing blood pressure fell from a mean of 154.7 mm Hg in the indapamide group to 134.4 mm Hg. Similarly, supine blood pressure fell from 155 mm Hg to 135.1 mm Hg. No such changes were seen in the placebo group.

Furthermore, 24-hour ambulatory monitoring showed that daytime mean systolic blood pressure fell from 149 mm Hg to 135.7 mm Hg and whole-day median arterial pressure dropped from 109 mm Hg to 98.7 mm Hg in the indapamide group.

In addition, there were no changes in a wide variety of biochemical parameters including serum sodium, creatinine, fasting blood glucose and insulin sensitivity. Dr. Kuo stated that, "the clinical utilization of indapamide SR in type 2 diabetes mellitus is safe and effective, not only in blood pressure control, but it also has no adverse effects on glucose metabolism and lipid profiles."
