The American Pharmacists Association’s Project IMPACT: Diabetes program helped communities with large numbers of diabetes patients attain reductions in A1C, blood pressure, BMI and other markers across socioeconomic and ethnic groups. The program pairs community pharmacists with diabetes patients for direct counseling sessions, and pharmacists also collaborate with other members of the patient’s care team. Interim data has shown mean decreases of 0.7% in HbA1c, 7.3 mg/dL in LDL, 1.9 mm Hg in BP and 0.2 in BMI. Endocrine Today Oct 30, 2013
