The use of insulin pens in most hospitals has gone back and forth more times than a tennis ball in a Williams sisters tennis match. It makes sense that having one pen for each patient would reduce errors and chances for cross-contamination, but the cost of throwing out unfinished pens can be a real deterrent. When you add in that a a lot of more experienced nurses prefer syringes, you can see why a battle ensues. This week in our Diabetes Disaster Averted, we look at a very unique problem that can occur with syringes that could have had a terrible outcome.

Eli Lilly’s cheaper, generic insulin lispro injection is now available for $137.35 per vial basis, or as a package of five pens for $265.20 based on list prices, although out-of-pocket costs can be lower depending on a person’s health insurance coverage.

A nurse mistook a standard tuberculin syringe for an insulin syringe and gave a patient 50 units of insulin instead of the prescribed 5 units. Sounds unlikely, doesn’t it?
