Comprehensive Diabetic Foot Exam (CDFE) Form and Pocket Chart
From the Australian National Association of Diabetes Centres and the Australasian Podiatry Council comes this CDFE form which adheres to the ADA Task Force document. The Pocket Chart is a good tool to have along with the exam form and was created using ADA data by the Southern Arizona Limb Salvage Alliance (SALSA), dedicated to care of people with diabetes and the reduction in amputations.
