I am sitting on my front porch after cleaning up from Hurricane Irma. We do not yet have electricity, but thanks to modern technology I can text, talk, and send emails. As we were helping our neighbor put their fence back up, I got a call from an older patient worried about their insulin and if they should not use their pen because the refrigerator was off. I advised them of the ability to keep a pen outside the refrigerator after it was opened. This simple question reminded me that everyone should have a plan for disasters, and this week we are reminded by our Medical Editor, Joy Pape, how we can all help others when disaster strikes.

The vent membrane in some infusion sets used with Medtronic’s insulin pumps prompted the company to initiate a recall of the affected sets, although no pumps or glucose sensors are affected by the recall. The components, which have already been discontinued by Medtronic, are prone to blockage by fluid during fill-tubing and priming, posing the risk of hypoglycemia and death.  Medtronic Recall

Lipoprotein metabolism in normal individuals and in type 2 diabetes mellitus: Triglyceride levels in blood are regulated by the size and number of VLDL secreted from the liver and the efficiency with which triglycerides are cleared from the plasma by lipoprotein lipases. In T2DM, greater numbers of larger, more triglyceride-enriched VLDL are secreted from the liver and lipoprotein lipase activity may be modestly reduced; both lead to increased levels of triglyceride in the blood. HDL cholesterol levels are reduced for several reasons, including increased transfer of that cholesterol to very low- and low-density lipoprotein, in exchange for their triglyceride, by cholesterol ester transfer protein. LDL particles lose some of their cholesterol to VLDL via the same exchange mechanism, which generate smaller, denser, LDL.
