Editor's Note

Last week our good friend from Diabetes Investor, David Kliff, provided us with his view on the future of insulin pumps, and all the companies that were trying to enter the market with their own pump. He seems to feel that in the long run the best product to use has the name Medtronic on it. This week our Publisher, Steve Freed, shares another part of his exclusive interview with Michael Hill, Vice President of Global Marketing at Medtronic for Intensive Insulin Management, as he explains the new Guardian systems.

DISASTERS AVERTED — Near Miss Case Studies
It is not unusual for people to have difficulty keeping insulin from freezing or getting overheated. A patient, with type 1 diabetes for 17 years, had glucose that did not respond to his rapid-acting insulin as it usually does. He had two new vials in the refrigerator. He took a new vial out of his refrigerator earlier in the day, and started using it a few hours after he took it out. Had high post prandials that did not respond as usual to correcting. He had enough experience to wonder if perhaps something was wrong with his new insulin, so he thought he’d try another vial. He saw it was frozen. He had put the two vials at the back, where for many refrigerators it is colder. He thought back and wondered if the first vial looked any different, but remembered, he did not look closely at it.

Pyruvate dehydrogenase and lipogenesis: The generation of pyruvate via glycolysis and L-PK brings the chapter full circle. Tracer experiments in humans estimate PDH flux to be very small in the postabsorptive liver. However, in the fed state activation of hepatic lipogenesis requires activation of PDH flux to generate acetyl-CoA from carbohydrate. PDH consists of three subunits (E1 – E3) that catalyze the sequential decarboxylation, acetyl-CoA formation and reduction of NAD+ to NADH, respectively.
