For those of you trying to enrich your vocabulary and understand the biological side of diabetes, you may want to look up the word mesenchyme. I was totally in the dark as to the meaning, and when I first read this week’s Clinical Text from International Textbook of Diabetes Mellitus, I found how important these mesenchymes are in the development and health of the beta cells of the pancreas. What was even more exciting was to learn that these cells may hold the key to preserving and rejuvenating beta cells in Type 1 patients.

This would be especially good news for our patient in this week’s Disaster Averted.

Met her for the first time, informed her she does not have to wear a pump. Some people are able to manage their diabetes as well or even better with injections. She wanted to try.

Pancreatic growth and epithelial branching morphogenesis: the extensively branched structure of the mature pancreas is attained by continued growth and remodeling of the primitive tubular epithelium in both dorsal and ventral buds. Once the buds have been induced, they are critically dependent on interaction with the mesenchyme for continued morphogenesis and cytodifferentiation.

A patient with diabetes gets her meds, prescribed by a nurse practitioner, from a national chain pharmacy in New York. She was planning travel to Hawaii. Patient did not refill her meds at home in New York before leaving for her travel in Hawaii because it was too early. Insurance would not pay until closer to the refill date. She thought she could wait until the time insurance would cover, go to a location where she was traveling and pick it up there. She’d done that during her travels in the past. It did not work in Hawaii.
