A study revealed diabetes patients who received care at a patient-centered medical home attained a 21% reduction in health costs at one year. Researchers also noted a 34% decrease in emergency department visits, an 18% drop in specialist visits, and a 32% cut in outpatient care costs. Researchers analyzed nearly 4,000 Independence Blue Cross members with diabetes. Half of those analyzed were treated in practices that were not PCMHs, and the other half were treated in PCMHs.
