About Diabetes-Meds

Diabetes-Meds is an organization dedicated to maintaining a high standard of care and preserving the health and well-being of senior patients in the United States.  This program is designed to educate patients, doctors, medical staff, pharmacists and others to utilize patient assistance programs set up by prescription drug manufacturers throughout the United States.  Pharmaceutical Companies have developed programs to assist low income patients maintain their health and well being by offering prescription drugs at either no cost or a small co-payment for the patient who qualify.  The problem, which prevents most medical offices from providing this service, is the red tape involved.  We know from experience that a large percentage of the senior population is not able to follow the directions given by their doctors due to the high cost of prescriptions medications.

Service Description

Diabetes-Meds provides a patient service to file pharmaceutical companies Patient Assistance Program (PAP) forms. The Service is designed for medical offices that do not wish to handle PAP program paperwork internally; this is typically a General Medical practice with a high Medicare population.  We work with the patient directly for services performed to submit PAP forms to the pharmaceutical companies.

The Process:

·   We provide the medical offices with mailing labels and patient information forms.

·   The patient will fill out the document and send the form to our office. When the patient requests our services by sending the form we are authorized to process the medical information provided.

• We will prepare all documents and return them for patient and physician signatures with a return addressed envelope.
• When the documents are received we will see that pharmaceutical companies process them.
• We charge the patient a uniform fee for document handling.

Our program is unique because we work one-on-one with the patient.

We keep the red tape to a minimum.

We keep all patient records confidential.

The drug companies send all medications directly to the doctor’s office or pharmacy.

To Qualify

The patient must be a U.S. Resident

Be able to show proof of income.

Make no more that $18,000.00 for a single person and $24,000 for a couple per year.
(these income levels are higher for some medications)

We will process your paperwork and locate your prescriptions immediately upon receiving your service fee.  We will keep patient information confidential. If your doctor is not familiar with the Patient Prescription Service we will explain the program.   We will do everything to make sure you get the prescriptions your doctor writes.  Our main concern is the patient.
