ABOUT THE PROGRAM

The Patient Experience Program (PEP) seeks to bridge research and educational programs to healthcare professionals, patients and their families through a collaborative effort to promote public health and to help people with Type 2 diabetes improve their quality of life.

The goal of the PEP is to encourage public responsibility in health management through a three-tiered strategy: education; intervention; and feedback. The PEP is designed to deliver:

 

·        Education on diabetes and its long~ term management through one-on-one patient interaction and targeted literature

 

·        Intervention through daily dietary supplementation with the clinically— substantiated Diachrome~ ingredient

 

·        Feedback from daily blood glucose monitoring

 

Each PEP kit will contain the following:

·        Patient education material on diabetes

·        Patient enrollment form

·        Program instructions: patient

·        Program instructions: healthcare professional

·        HbA1c test -2
