2)
· One Professional CPN program per medical professional
· Study duration per medical professional will be two
weeks to initiate the study (program installation and medical
professionals instruction in its use), six weeks to process
study patients, one week to submit study data to DiabetesInControl
for delivery to NCC

who have diabetes
· Area with desktop computer (modern Windows PC with
sound system and printer) suitable for patient instruction
(provides adequate privacy with protection of patient health
care data)

using a computer-based validated instrument
o Patient goes through the Patient Interview process with
assistance from a clerical person if necessary
o Patient is weighed

with the patient, the plan and four weeks of menus are given
to the patient, medical professionals will decide on carb

log (medications taken, changes to menus, exercise performed)
o Patient treatment plan and menus will be filed in a computer
folder
o Medical professionals will prepare and file a SOAP note
including the following data

· Second patient encounter (four weeks later)
o Repeat accessing patient knowledge of and attitudes about
diabetes using a computer-based validated instrument

o Patient will perform a computer based evaluation of the
CPN program (general satisfaction, worthwhile doing, want
to continue using the program materials, problems encountered,
suggestions for improvements)
o Patient will be weighed
o Medical professionals will perform a computer based evaluation
of usefulness of the CPN program with the current patient
(was patient health status improved)
o Diabetes log will be mailed to NCC, the patient will be
only identified by their study number
o
· Completion of study
o Medical professionals participating in the study will perform
a computer based evaluation of CPN program (general satisfaction,
worthwhile doing, want to continue using the program materials,
problems encountered, suggestions for improvements)
o The patient treatment plans, the results of the two patient
interviews and the medical professionals interviews will be
sent to NCC as an attachment to an e-mail
