Patient Selection Data – Infusion Device Study

Thank you for enrolling in the infusion device study. In order for us to select the proper candidates for the study please read the inclusion and exclusion criteria and then answer the following questions.

Inclusion criteria:
Has been diagnosed with insulin requiring diabetes (Type 1 or Insulin dependent type 2)
Usage of continuous subcutaneous insulin infusion (CSII) therapy for a minimum of 6 months
Must be willing to participate in the full study.
Will understand and sign a written informed consent, or a parent or legal guardian will understand and sign.
Will understand and sign a written non disclosure agreement, or a parent or legal guardian will understand and sign.
Will be available for 2 calls from Diabetes in Control, 1 at the beginning of study to insure proper use and one at end of study for verbal feedback.

Exclusion criteria:
Users of the following insulin pumps manufactured by Medtronic Minimed: Paradigm pump 511 and Paradigm pump 512(the device will not fit)
Is incapable or unwilling to sign a written informed consent and non disclosure agreement
Has a history of several glycemic incidents when using an external insulin pump
Has any other complicated illness/disease which is believed will make it difficult for the participant to complete the study, as seen by the participants medical professional/educator
