The purpose of this
experience survey is to gather information for MSI as to the value of
Formula II Skin Care Cream for your patients
About MSI
This
Experience survey is not being used to evaluate changes in blood glucose
readings and does not require any extra testing.
Our
patients often come to us for product recommendations. We all try to
deliver information to our patients but often we have no experience with
the products we recommend.
We
have arranged for MSI to
provide you with the following product for your patients use at no charge.
Formula
II (Premier) Skin Care Cream
They
are interested in your feedback and suggestions.
This
survey will last for 60 days.
We
will be providing MSI with
results of the survey and any comments- only shipping information about
each individual educator will be provided, no list of participants be
given to them.
Registration:
At
this time we are seeking medical professionals who will use this products
with their patients and complete the 30 and 60 day survey forms.
Honorarium-
Each Educator will received a gift package of MSI
products for
their participation.
