I have been working with a group of residents at a local hospital going over insulin dosing: 9 residents answering 10 dosing questions each. The wrong dose was given 65% of the time and 1/2 of those would have caused harm to the patient. Now Novo has put together an explanation of why and how these errors occur. Check out this week’s Disaster Averted to get the whole story.
