For years we have had methods for titrating patients with long-acting insulin and there have been very successful patient self-titration algorithms that have proven to work as well as physician-directed dosing. The problem has always been what to do when we need to begin mealtime insulin. Since the use of rapid insulin can often lead to increased events of hypoglycemia, we are often reluctant to start a patient on it and will increase basal rates to very high levels.
