For years we have had methods for titrating patients with long-acting insulin and there have been very successful patient self-titration algorithms that have proven to work as well as physician-directed dosing. The problem has always been what to do when we need to begin mealtime insulin. Since the use of rapid insulin can often lead to increased events of hypoglycemia, we are often reluctant to start a patient on it and will increase basal rates to very high levels.

There is now a way to overcome the fear of hypoglycemia and have your patients self-titrate based on results from the AUTONOMY trial. This week we have a Homerun Slides series from Dr. Steven V. Edelman et al. for you that shows how patients can successfully self-titrate rapid-acting insulin
