I just finished a program in Danville, California, on hospital readmissions where I spoke to medical professionals about the high cost of readmissions and possible ways to avoid them. The program focused on using care and case managers to properly help patients leave the hospital, avoiding costly and sometimes deadly readmissions. This topic points out how much better a job we need to do with our patients. In fact, if you look at this week’s Diabetes Disaster Averted you will see how important this care management is and how the transfer of vulnerable patients from hospital to long-term care can be devastating if proper care guidelines are not followed.
Our Clinical Text covers the use of macronutrients for type 1 patients and in our Homerun Slides, Dr. Stanley Schwartz continues sharing his perspectives and solutions for the obesity and diabetes problems that all our patients face. Our Mastery In Minutes video also has some tips for you on Nutrition and Fitness Medicine from Dr. Carol Wysham.
