When patients present themselves in the ER with severe hyperglycemia, it is very important to determine if the patient is in severe ketosis. Many patients that we think would have high ketone levels don’t and the reason could be hyperosmolar non-ketotic hyperglycemia. This needs to be determined early in the ER as treatments for these patients are actually the opposite of what you would expect.
