The app includes digital coaching designed by a CDE with blood glucose tracking; medication tracking; feedback; and reminders. Additional tools include a carb tracker; barcode scanner; hundreds of recipes; sample menus; and tool to get active. Daily support involves message boards and daily tips. Patients can also share their data with clinicians. For an extra charge ($3.99), patients can get daily menus of diabetes-friendly recipes taking the guesswork out of healthy eating.

This app just came on the market (January 14) and is only available in iOS with an Android version on the way. The few reviews in so far have been positive.
UPDATE (August 12, 2013): not recommended as there seems to be no support for tech problems.
