The correct answer is 4. 8u of Novolog (4) 8u of Novolog.

  • Correction Factor 1 unit will lower BG 30 mg/dl
  • Correction Goal Pre- Meal Goal 120 mg/dl
  • Carb Ratio 1unit will cover 15 carbs

So if the patient’s pre lunch reading is 270 mg/dl and she is going to eat 90 carbs what dose of Novolog would they need?

  • Dose for Correction: 5u
  • Dose for Carbohydrates: 6u
  • Total Dose of Novolog: 11u

But since she took a correction 2 hours earlier, then according to most studies, the on board insulin still has 44% still working and must be accounted for or she will increase her risk for hypoglycemia, so we must deduct 44% of her correct dosage. The correct dosage without considering the onboard insulin is 11u x .44 = 3.08 units.
11u – 3u = 8units of novolog.

Studies have shown that:

  • After 1 hour 75% of bolus remaining
  • After 2 hours 44% of bolus remaining
  • After 3 hours 22% of bolus remaining
