Editor's Note: Paying Now or Later

When Steve and I put this week's Clinical Mastery Series issue together I kept thinking of the old Fram oil filter commercials: the ones where the mechanic came on and said, "You can pay me now or pay me later," referring to the relatively small cost of replacing an oil filter as opposed to the much higher cost of replacing an entire engine. This old advertising slogan sums up the information we have for you.

When Dr. Edelman discusses the use of pramlintide in patients he is really focusing on a paradigm shift in care. Since the discovery and use of insulin began the goal has been to fix the problem of too much glucose in the blood stream. This method of treatment overlooked the reasons for the appearance of glucose in the blood altogether but now with the use of pramlintide we can actually decrease the glucose. Our Diabetes Disaster Averted shows the value of making sure calculations are correct before the problem occurs and even the Homerun Slides point out the value of using more than just a fasting reading to diagnose and treat hyperglycemia.
