CASE 2 – Efficacy and Benefits of GLP-1 Receptor Agonists

Paul is a 53-year-old white male who has been diagnosed with type 2 diabetes mellitus (T2DM) for about 2 years.

He is currently taking metformin 1000mg twice daily and glimepiride 4 mg once daily. Soon after diagnosis and beginning antihyperglycemics, Paul's glycosylated hemoglobin (A1c) showed improvement. His glycemic control worsened over subsequent years, and at a recent office visit his A1c was up to 7.8%. Paul has experienced several episodes of hypoglycemia in the last couple months. He has also continued to gain weight since diagnosis, with a current BMI of 35 kg/m2 and waist circumference of 43 inches. His most recent cholesterol panel was LDL 170 mg/dL, TG 200 mg/dL, HDL 45 mg/dL. He is currently taking simvastatin 20 mg daily.
