 Diabetes 24/7 is a new application using personal health records that will allow people to manage their condition and share valuable health information with others on their care team. It is designed to provide users with tools to help monitor their diabetes and to track data such as blood glucose patterns, laboratory results and physical activity sessions. The application will integrate with Microsoft's HealthVault program and utilize its data sharing capability. The application serves as a portal for personal health information and provides the ability to transmit and share data from HealthVault enabled devices such as pedometers, blood pressure monitors and blood glucose meters, to aid in the management of diabetes.
