The purpose of this experience survey is to gather information for Pritchett & Hull as to the value of their handouts in your practice  

This Experience survey is not being used to evaluate changes in blood glucose readings and does not require any extra testing. 

Our patients have a lot to remember…. how to monitor, what to eat, when to exercise, how to avoid complications and much more.

We all try to deliver this important information to our patients but often it is too much for them to remember.  

We can have them take notes, we can hand them a written form, or we can give them professionally prepared, simple to understand handouts that mix text, graphics and pictures.  

Pritchett and Hull has been producing high quality patient education materials for over 28 years, and they would like your feedback on three of their diabetes tear sheets.    

We have arranged for Pritchett and Hull to provide you with the following handouts at no charge  

Carbohydrate Counting Tearpad, Item 349 

Diabetes Exercise Plan Tearpad, Item 413  

Foot Care Tearpad, Item 269 

They are interested in your feedback and suggestions.

We will be providing Pritchett and Hull with results of the survey and any comments- no information about each individual educator will be provide, nor will a list of participants be given to them.  

Registration: At this time we are seeking medical professionals who will use these handouts with their patients and complete the 30 and 60 day survey forms
