Is the meter right and do expired strips make a difference?  On Friday I saw a new insulin dependent patient for diabetes outcome management. She was coughing and sweating and kept having to drink water. I had done an instant A1c and it was 8.6 She stated that her glucose was 235 right before I got there.  I had brought her a new Precision Ultra meter, and when I took her glucose it was 346. We tested against her meter and it was 244, a 101mg/dl difference.  We did it again and the disparity was similar. Not knowing which meter was correct I got a third meter and tested, the reading was 343 mg/dl. When I checked her meter the strips had expired 13 months prior and the code was off by 2. We had to re-adjust her insulin….. Have you compared your patient’s meter to another lately? It might be worthwhile. Remember that some meters measure whole blood and some measure plasma, which will be higher.
